To request a copy of your medical record please download and complete the Medical Record Authorization Form.
Information will not be released without the patient's written consent, except as required by law.
Forms should be emailed to [email protected] along with a valid picture ID (required) or Fax to 615-997-1200.
The Recovery Center is now closed as of 6/3/2025 and any records available are stored offsite and not readily available.
Requests can take up to 30 days to process. All requests will be processed as timely as possible.
